Request for New PAN Card or / and Changes or Correction in PAN Data Form

Instructions:-
1. Two passport size colour photos. (the image in photo must  be very clear and xerox copy of photos will be rejected. All colour Xerox photos are unacceptable.
2.Photo ID Proof like passport copy, election card, dirving license or any such proof. (See Instructions)
3 Address Proof, if the current address is different from the one in the photo id proof.
 bank statement, telephone bill etc. (See Instructions)
If the applicant is not having any photo id proof or the address proof then he is required to sign and also get the attestation done from the gusseted officer on the below affidavit and submit it along with application.

(a) Applicants may obtain the application form for PAN (Form 49AA) from TIN‐Facilitation
Centres (TIN‐FCs) / PAN Centres, Depository Participants (DP), any other stationery
vendor providing such forms or download from the TIN website (www.tin‐nsdl.com).
This duly filled Form 49AA along with the prescribed documents is required to be
submitted to your DP.
(b) The fee for processing PAN application is Rs. 85/‐ (plus service tax, as applicable).
(c) Those already allotted a ten digit alphanumeric PAN shall not apply again as having or
using more than one PAN is illegal. However, request for a new PAN card with the same
PAN or/and Changes or Correction in PAN data can be made by filling up 'Request for
New PAN Card or/and Changes or Correction in PAN Data' form available from any
source mentioned in (a) above. The cost of application and processing fee is same as in
the case of Form 49A & Form 49AA.
(d) An acknowledgment containing a 15–digit unique number will be issued to the DP
concerned on acceptance of this form. This acknowledgment number can be used for
tracking the status of the application.

Application for allotment of New PAN (Form 49A) – applicable for Citizens of India

With effect from April 8, 2012, PAN applications are required to be furnished in the new forms prescribed by ITD. Indian citizens will have to submit their ‘Application for allotment of new PAN’ in revised Form 49A only. Foreign citizens will have to submit their ‘Application for allotment of new PAN’ in newly notified Form 49AA only.
With effect from April 1, 2012, fees for PAN application has changed to 96. (For dispatch outside India 962).
For New PAN applications, in case of Individual and HUF applicants if Address for Communication is selected as Office, then Proof of Office Address along with Proof of residential address is to be submitted to NSDL w.e.f. applications made on and after 1st November 2009.
