The Government of Andhra Pradesh is committed to provide Simple, Moral, Accountable, Responsive and Transparent (SMART) services to the people through the concept of Citizens' Charter. The Transport Department, as part of the implementation of this Citizens' Charter, has simplified the procedures to get a Driving Licence.

SPECIAL PROGRAM

For obtaining a Driving Licence, one can approach the offices of Transport Department on all working days. Apart from this, the Government has initiated a special program for the convenience of the public. Accordingly, the Transport Department has made special arrangements on Monday and Tuesday of every week and also on every fourth Saturday of the month from 9 a.m. to 1.30 p.m. to issue Driving Licences.At present, this program is for issue of Non-Transport Driving Licences only. In this special program, the candidates are requested to approach the officers concerned directly, as the agents are not allowed to represent the candidates. The entire public is requested to avail these facilities being provided by the Government of Andhra Pradesh.If the public face any difficulty in obtaining a Driving Licence, they are requested to approach the Deputy Transport Commissioner (DTC)/Regional Transport Officer (RTO) of the concerned district or the supervisory DTC of the region. Why do you need a Driving Licence?
As per Motor Vehicle Act 1988, a valid Driving Licence is necessary to drive any motor vehicle on public roads

Where do you get it?At the Regional Transport Office or Motor Vehicles Inspector's Office, having jurisdiction over your residential area.
When do you get it?
After completion of 16 years of age, for driving motor cycles with engine capacity not exceeding 50cc with the consent of the parent / guardian.After completion of 18 years of age, motor cycles with engine capacity exceeding 50cc and light motor vehicle. After completion of 20 years of age, for an endorsement to drive transport vehicles.
How to get a Driving Licence?
There are two stages to get a Permanent Driving Licence.

Do you need to produce a Medical Certificate?
Applicants for the Non-Transport Licences, under the age of 50 years do not require a Medical Certificate. However, those applicants who are over 50 years of age must produce a Medical Certificate.

All applicants for Transport Vehicle Licence must produce a Medical Certificate, irrespective of their age.
How do you obtain a Learner's Licence?Applicants for a Learner's Licence should appear personally before the Licensing Authority with the following:
  • Application in Form No. 2
  • Fees of Rs. 30/- for each class of vehicle
  • User Charge of Rs.30/-
  • Proof of age, appropriate for relevant class of vehicle.
  • Proof of residence like Ration Card, Electoral Roll, Electricity / Telephone Bill etc., showing the name of the person.
  • Application-cum-declaration as to the physical fitness in Form-1
  • Medical Certificate in Form No. 1A wherever applicable.
  • 3 recent passport size photographs of the applicant.
  • Applicants for Transport Vehicle Licence must produce a Permanent Driving Licence for Light Motor Vehicle held by him, at least for one year.
Applicants for Learner's Licence shall pass a computerized  test regarding basic traffic signs and driver's responsibilities.

The Learner's Licence so issued is valid for a period of six months from the date of issue.
    How do you obtain a Permanent Licence?The applicant should appear in person along with a registered motor vehicle of the relevant category before the Licensing Authority with the following:
  • Application in Form No.4 (available with the department) 
  • Fee of Rs. 35/- for test and Licence 
  • Valid Learner's Licence held by the applicant for the relevant class, which is older than 30 days 
  • 4 recent passport size photographs of the applicant 
  • Applicants for Transport Vehicle Licence should enclose a Training Certificate in Form No. 5 issued by a recognized driving school.
The applicant shall pass a driving test on a vehicle of the type he has applied for.

How long is the Driving Licence valid?
Licence to drive a Non-Transport Vehicle is valid for 20 years from the date of issue or until the holder attains the age of 50 years, whichever is earlier. After that, the Licence is renewed every 5 years.Transport Vehicle Licence is valid for a period of 3 years from the date of issue or renewal.

10th class Re-verification application form

1. The candidates who want to apply for reverification of answer scripts have to apply in
the prescribed format for the subjects, I language, II Language, III Language,
Mathematics and General Science only.
2. Application format is kept in the website www.bseap.org
3. The prescribed fee for each subject is Rs.1000/-
4. The Xerox copy of the valued answer script will be sent to the candidate after reverification.
5. The filled in applications have to be submitted in the counters opened for the purpose
in the respective DEO’s offices only.   No applications shall be entertained which will
directly sent to this office by post.
6. The applications for Re-verification will be accepted up to 06-06-2012.
7. The candidate has to forward the application through the head master of the school
concerned duly getting the photo attested which is affixed on it.
8. Candidates who applied for reverificaiton of marks need not apply for recounting of
marks.
9. One Self addressed envelope of  12 X  9 ½ (book size) without stamps  and another
cover of 10 X 4 ½ with the address of the head master concerned.
10.The candidate has to enclose the Xerox copy of the hall ticket and dummy marks
memo otherwise the application will not be accepted.
11.The prescribed fee has to be remitted through challan into the following head of
account.

12.Fee paid by way of Demand drafts and bankers cheques will not be accepted under any

a) Re totaling.
b) Whether marks for all answers are posted or not.
c) Unvalued answers.
15.The Challans have to be paid by the individual candidates only and no group challans
will be accepted.
16.Application for  recounting will be accepted in the O/o Director of Government
Examinations only.
17.There is no reverification facility for social studies.

Documents to be Submitted Along with the Application



Proof of Identity and Address
·         Documents which are acceptable as proof of identity and address are mentioned in detailed instructions for documents to be submitted.
·         Proof of Identity and address must be in the full expanded name of the applicant as mentioned in the application form.
·         If the applicant is a minor (i.e. below 18 years of age at the time of application), any of the documents acceptable as proof of identity and address of any of the parents/ guardian of such minor shall be deemed to be the proof of identity and address of the applicant.
·         Proof of address in the name of spouse/parents/sibling etc will not be accepted as valid proof of address.
·         Proof of Address is required for the address mentioned in "Address for communication" in item no.7.
·         If item no.8 is ticked & filled in, proof of address mentioned therein is also required.
Proof of PAN
·         Proof of PAN is required for the PAN currently in use (i.e. existing PAN). This is required for the PAN mentioned at the beginning of the application before item no. 1
·         Proof of PAN is also required in case of cancellation request for any PAN, i.e., for any PAN mentioned in Item No. 10
·         Proof of PAN can be one of the following only:
o    Copy of PAN card; or
o    Copy of intimation letter issued by the Income Tax Department in lieu of PAN card intimating PAN.
o    In case one of the above proofs are not available, a copy of FIR (stating loss of PAN card) can be submitted.
If proof of PAN (as stated above) is not submitted, the application will be processed on a 'good effort' basis even without a copy of FIR. During verification at NSDL, if it is found that there are differences between the PAN or the data provided in the application with the ITD database, the application may not be processed and the processing fee will be forfeited.
Proof for Change Requested
The applicant shall be required to provide further documentary proof to support request for correction or change in PAN data like name, father's name, date of birth, for example:
·         Request for change (marginal correction like spelling correction, expansion of initials, etc.,) in applicant's or father's name will have to be supported with suitable proof of identity containing corrected data.
·         Request for change (significant change) in applicant's or father's name will have to be supported with such proof that will contain proof of change of name from the old to the new in addition to the proof of identity. The documents that shall be accepted as proof in this case are:
o    For married ladies - change of name on account of marriage - marriage certificate, marriage invitation card, publication of 'name-change' in gazette, a certificate from a gazetted officer stating name change, copy of passport showing husband's name.
o    For individual applicants other than married ladies - publication of `name-change' in gazette, a certificate from a gazetted officer stating name change.
o    For companies - ROC's certificate for name change.
o    For partnership firms - revised Partnership Deed
o    For other categories which are registered organisations (AOP/Trust/BOI/AJP, etc.) - the revised registration/deed/agreement.
·         Request for correction of date of birth (for individuals) and date of incorporation (non-individuals) will have to be supported with documents issued by competent authority containing the correct date.
·         The address for communication in the ITD database will be updated with the address for communication mentioned in the application, even if change in address for communication is not requested in the application.
If the core data relating to the PAN (the applicant's name, date of birth, father's name (for individual), name and date of incorporation [for non-individual]) provided in the application substantially varies with the PAN data in the ITD database and the applicant has not requested for the change in the same with sufficient support documents, the applicant will be intimated of this variance. In such case the applicant will have to provide necessary support documents.

7. Address - Residential and Office
R - Residential Address:
It is mandatory for Individuals / HUF / Body of Individuals / Association of Persons and Artificial Juridical Person to mention residential address and provide proof of the same. The name of applicant mentioned in the application should match with the name displayed in the proof of address.
Out of first four fields, applicant must fill up at least two fields.
Town/City/District, State/Union Territory and PIN are mandatory.
If the applicant wants to enter foreign address here, option for "outside India" should be selected from the drop down list in the field of "State/Union Territory". Applicant should select the name of the country and enter the ZIP code as applicable. Country name and Zip code are mandatory.
Applicants other than Individuals / HUF / Body of Individuals / Association of Persons and Artificial Juridical Person will leave this field (Residential address) blank.
O - Office Address:
In case of Individuals / HUF / Body of Individuals / Association of Persons and Artificial Juridical Person, if Item No.6 (Address for Communication) is selected as "O" then Office Address is mandatory.
Office name and address is mandatory in case of Individuals having source of income as salary & business/profession.
In case of Firm, Company, Local Authority and Association of Persons (Trust), name of office and address is mandatory.
For all categories of applicants, out of first four fields, at least two fields are mandatory.

'R' means Residence and 'O' means Office.

9. Telephone Number and e-mail ID
Telephone number should include country code(ISD code) and STD code or Mobile No. should include Country code(ISD Code).

NRI /Foreign National should mention the ISD code of their respective country and City code as applicable in the space provided for ISD & STD code. e. g. Person staying in Chicago should write A1 in the ISD code and 312 in STD code text box. (A1 is ISD code of USA and 312 is City code of Chicago).
It is mandatory for the applicants to mention either their “Telephone number” or valid “e-mail id” so that they can be contacted in case of any discrepancy in the application and/or for receiving PAN through e-mail.

Application status updates are sent using the SMS facility on the mobile numbers mentioned in the application form (applicable only for Indian mobile numbers).
It is suggested that applicants mention their telephone number so that they can be contacted in case of any discrepancy in the application form.

10. Status of Applicant
This field is mandatory for all categories of applicants.In case of ‘Limited Liability Partnership’, the PAN will be allotted in ‘Firm’ status.
11. Registration Number
Not applicable to Individuals and HUFs. Mandatory for 'Company'. Company should mention registration number issued by the Registrar of Companies. Other applicants may mention registration number issued by any State or Central Government Authority.
12. Country of citizenship & ISD code of country of citizenship
It is mandatory to provide country of citizenship and ISD code of country of citizenship. Please select country name and respective ISD code from the list of country and ISD codes
13. Source of Income
It is mandatory to indicate at least one of the sources of incomes, as mentioned in the form. In case, the income from Business/profession is selected by the applicant then an appropriate business/ profession code should be mentioned.

14. Name and address of Representative Assessee
Section 160 of Income Tax Act, 1961 provides that any assessee can be represented through Representative Assessee.
Therefore, this column should be filled in by representative assessee only as specified in Section 160 of the Income-tax Act, 1961, such as, an agent of the non-resident, guardian or manager of a minor, lunatic or idiot, Court of Wards, Administrator General, Official Trustee, receiver, manager, trustee of a Trust including Wakf.
This field will contain particulars of the Representative Assessee. This field is mandatory if applicant is minor, deceased, idiot, lunatic or mentally retarded. Column 1 to 13 will contain details of person on whose behalf this application is submitted.
Proof of Identity and Proof of Address has to be submitted for the Representative Assessee details mentioned.
Foreign Non-Individual entities having no office of their own in India may additionally mention their own name and care of address of their tax consultant in India in this column for the purpose of communication address. This should be supported by Power of Attorney in favour of the tax consultant duly notarised or attested by Indian Embassy/High Commission or Consulate located in the country.
Name of Representative Assessee should not be prefixed with titles such as Shri, Smt, Kumari, Dr., Major, M/s etc.
15. Proof of Identity and Address
Select appropriate proof of identity and address from the relevant drop-down list provided in the form.
Documents submitted should be in the name of the applicant as mentioned in item no. 1 of the application form.
In case of minor, proofs of any of the parents or guardian shall be deemed to be the proof of identity and address.
For HUF, an affidavit made by the Karta of Hindu Undivided Family stating name, father’s name and address of all the coparceners on the date of application and copy of Proof of Identity and Proof of Address documents in the name of Karta of HUF is required.
16. Other Details: Depository Account
If applicant selects 'copy of depository account' as proof of identity or proof of address, it is mandatory to fill in Depository Account Details.
17. Payment Details
An applicant has an option of making payment either by demand draft, cheque, Credit Card / Debit Card or Net Banking (Payment details are mentioned in guidelines).
18. KYC details
Please note that KYC details are required to be filled by Qualified Foreign Investors (QFIs) and Foreign Institutional Investors (FIIs) only. QFI and FII applicants should submit their physical application form to their concerned Depository Participant (DP).
19. Signature/ Left Thumb Impression
Application must be signed by (i) the applicant; or (ii) Karta in case of HUF; or (iii) Director of a Company; or (iv) Authorised Signatory in case of AOP, Body of Individuals, Local Authority and Artificial Juridical Person; or (v) Partner in case of Firm/LLP; or (vi) Trustee; or (vii) Representative Assessee in case of Minor/deceased/idiot/lunatic/mentally retarded.

It is mandatory to provide Verifier’s name, capacity and place in the application.
